All containers must be non-leaking and properly sealed prior to pickup. If you have leaking or non-sealable containers, please contact EOHS at x6866 immediately.

In order for your information to be properly submitted, you MUST enter the number of containers.

"Unknown" or "Other" selections must be adequately described in the Comments field.

By clicking on the submit button, you are certifying that the information submitted is accurate and that all waste containers are labeled to correctly identify their contents. Improper disposal of biological/infectious waste is a violation of State and Federal law.

If you have more than 5 items or multiple locations, enter your first 5 items, click submit and follow the instructions for adding additional items.
